I think you are looking at the By Category view of the Contacts folder.
You
won't see al Categories as headings in this view.

The Contacts will be grouped according to their Category. If no Contacts
is
in the Category "Trainer" for example, you will not see that Category at
all.

Maybe your view is corrupted, it's possible to reset views -
