This happens with most of the emails he sends to print, but
intermittently it works correctly.
Printing directly from MS WORD works fine, as does printing from other
applications.
I'm looking for settings in all related apps, and spent 3 hours on
the phone with Microsoft support, but no luck yet.
Does anyone here recognise this? What is the fix?

Check under ViewEncoding that it is set to Western European (Windows) or
Western European (ISO). (Mine is ISO).
Check also under ToolsOptions for settings in your preferred Read/Send
and that it is set at MIME/None and NOT MIME/Quoted Printable.
