Default unable to use customized form

When I click on the contacts folder and select New I get that message that
says it can't find the form and contact our Administrator.

I don't want to create a new form, it took me hours to create the one I
already have.

Thank you!

That still doesn't tell us exactly what you're doing that results in the error message. Also, can you create a new item using the Tools | Forms | Choose Form command and picking the form from your Personal Forms library?

If I look at the contact folder in my list view, I right click on the
contacts form and go to properties then select the form from "when posting to
this folder use:". The form is stored under the personal forms library. I
believe this is happening because I don't have it saved in the correct area.

Thank you!

What exactly are you clicking on? An item you created with the form? Where is the form published? Where is the item stored?
