Default Edit existing Outlook form

In Outlook 2003 and later, developers will see a different layout when they put a contact form into design mode. The built-in layout is not available to be customized, except with Outlook 2007 form regions.

For other contact form idiosyncracies, see http://www.outlookcode.com/d/contactform.htm

Please help: What I am trying to do is to expand the default Contacts form
and add some fields on the left of the form , right next to the big text box.
I open the form, choose Tools-Forms-Design this form and start to add what
I need. Unfortunatelly when I save the already changed form, it looks a
little bit different - some elements do not present at all anymore, and some
of them are additionally added! Also the buttons look really ugly - the
original buttons are rounded and the new ones are quadratic

So - how can I keep the look of the default form and, of course, my newly
added fields? Please, anyone help! I am clicking around already a lot of time
without success

Here are two links to the screenshots with the differences enclosed:

Thanks in advance
