Default Survey Form Doesn't return the form fields

It's completely unsuitable for an external customer survey. The only way to make that form work in your environment is to publish it to every user's Personal Forms library with the "send form definition with item" option turned off.

If you also have Access 2007, a better solution is to use its External Data | Collect Data command to generate HTML-format messages that can be used to collect survey results.
