I had the same problem some months ago and ended up redoing the form and it
worked, however as of yesterday we are having the same problem again Ahhhh.
All staff are recieving normal blank email with no customisation. However the
infomration is there as we have inserted a field in Outlook (field chooser)
which shows up one of the fileds of the customised pone message form (Name of
person calling). But when you open it is just a normal blank email message.
Please help as we have several customised forms which the company relies on
daily and done are working!

Also, if you are using Microsoft Exchange, the best location to publish a
form that needs to be accessible to all users is the Organization Forms
library, or the Folder Library for forms not based on the standard e-mail
form.
