Default Drop down calendar

I'm designing a form in which the user needs to enter dates in several fields
and to make it easy I'd like to users to be able to click on a drop down list
and select a date from a calendar (also with time) - just as you can when
entering the date and time of a future appointment in Outlook. I just can't
find any indication how to do this. Can anyone help?
