By "when it is run," do you mean when the user creates a new item with the form? Where is the form published?
By blank, do you mean that there is no data in controls that should be showing data?
FYI, there is a newsgroup specifically for Outlook forms issues "down the hall" at microsoft.public.outlook.program_forms or, via web interface, athttp://www.microsoft.com/office/community/en-us/default.mspx?dg=micro...

It's back...
At least the problem with the blank form.
I created this form on a WinXP/Outlook 2003 machine... when it is run
on a WinXP/Outlook 2003 machine, the form is BLANK.
What setting am I missing?
thanks!

Thanks for the pointer about the separate site. I will check that out.
It is blank - meaning no controls. Some of the computers are fine,
others are not. When I say "run," I mean the OFT file is clicked on,
initiating Outlook.
thanks!
