What happens if you start Outlook in its safe mode ("outlook.exe
/safe") to eliminate loading any installed add-ons to Outlook and then
exiting Outlook? If that works then an add-on is your problem.

If an add-on is your program, have you yet right-clicked on the
dccmsp32.dll file to see who is listed as the author and perhaps seen
a program title under the Version tab for properties on that file. I
have that file and it appears to be from Symantec for Winfax. If
Outlook worked okay in its safe mode, see what happens if you disable
the Fax Server add-on in Outlook, exit Outlook, reload Outlook, and
then check what happens when you exit again.
