Outlook doesn't have an address book. The "address book" is just a view of
your contacts in your Contacts folder and it's controlled by having
correctly configured thr Outlook ADdress Book service in your mail profile.
It appears you've failed to do that. See this:

2. when I tried to open address book to create new contact name, I
received the following error "the address list could not be
displayed. The Contacts folder associated with this address list
could not be opened; it may have been moved or deleted, or you do not
have permissions, For information on how to remove this folder from
the Outlook Address Book, see \Microsoft Office Outlook help.". This
is a new installation, why this error. Also I am the only one who
logon to this machine.
Fix the OAB service and this will be addressed.
