What if you have changed their document and want to reply.
Do i have to first save it, rework it, then upload it again?

Copy and paste does work, but when I press the reply button on an
e-mail with an attachment, shouldn't the new reply automatically have
the attachment placed in it by outlook? Is there some switch or
setting to make it automatic?
No. Outlook assumes that whomever you're replying to sent the
attachment to you and thus doesn't need to get it back.
