Usually for time differences or other identifiers which could indicate a
unique new email being send. A common issue on the sender side is that it
hangs in an outgoing mail virus scan and gets send multiple times.

Plenty of things. Could even be sender side. If it shows up twice in OWA
it is probably a sender related issue. Check the header info for more
details.
