Likely causes:
-- You didn't publish the form to the Organizational Forms library on the company's Exchange server.
-- You did publish to Org Forms but checked the "send form definition with item" box on the (Properties) page.
-- You did not click the Edit Read Page button and create a read layout with controls to match your control layout.

This is my first time designing a custom form in Outlook. I am trying
to create a form that users can use to trade stocks. I started with a
message form and then customized to by deleting the body section and
inserting the necessary fields for the users to populate (symbol,
amount, etc). However, when i open the form to test it, the message
that appears in my mailbox is the defaulted message form (as if i sent
a regular email). I have been able to find any assistance on the
internet. Any help would be greatly appreciated.
