I'm not aware of any concrete reasons why the VBA Add-In in particular would
be disabled, but this can occur if other installed add-ins - or present
macros - malfunction.

Try turning off other add-ins or comment out any suspect macros to see if
you can isolate the problem.

You receive an "Outlook failed to start correctly last time" error message
when you start Outlook:
