If outgoing messages remain in your Outbox (they never go "straight to Sent
Items" - they always pass through Outbox first and then move to Sent Items
once they've been sent), that means that Outlook doesn't get an
acknowledgement from the outgoing server that it has received the mesage.
Enable diagnostic logging and see what's happening between Outlook and the
server.
