right click on an empty spot of the toolbar area - choose customize. Then
with the customize dialog open, r-click on the New button. Is image and text
selected?

you can also close outlook, find outcmd.dat and delete it. Restart outlook
and it'll rebuild the toolbars, removing all customizations.
