Default Do we need to be running exchange server for shared calendars etc

if you don't use exchange, you need another server - see
http://www.slipstick.com/outlook/share.asp

exchange should be doing everything - you don't need mail hosted outside -
and even if you decided to keep it outside, you could easily configure
exchange to handle both the outside mail and calendars seamlessly.
