Default Outlook 2003 won't allow identity selction on startup

Outlook doesn't have identities it has Mail Profiles. Not prompting for
which profile to use is a sign that Outlook isn't closing properly. Use
Task Manager to make sure the OUTLOOK.EXE process is gone. Reasons why
