Default Professional Designations

Can anyone tell me if I can include a professional designation or multiple
designations, e.g., M.D., with a contact name and have other fields populate
properly? If I enter "John Smith, M.D." as the full name, then the 'File as"
field displays by first name rather than last name, and the e-mail "Display
as" field shows "M.D. John Smith," which makes it next to impossible to find
the name when I am creating a new e-mail.

If I omit the comma (for a single designation), the "Display as" field still
does not file by last name but by first name.

Also, entering multiple designations and using a comma to separate them does
not work properly either.

Any suggestions?
