How are these forms published? To a folder Forms Library, Personal Forms
Library, or Organization Forms Library? In most cases when you create a new
Outlook item from the default New - [Item Type] command, it launches a
default form. Custom forms are usually launched from the Actions menu,
especially when they are published to a folder Forms Library and that folder
is currently active.

Custom Outlook forms (calendar events) created for Outlook 2003 are not
working in full/fresh installations of Outlook 2007. However, if someone
does an upgrade from 2003 to 2007, it seems to work fine. Anyone know
what
could be left on the upgrade systems that forces it to work? The forms in
the non working (fresh install) are there, you can see them, but dont come
up
with a new appointment.
