Since you didn't mention what operating system is involved in the equation,
the first and formost things to check a

1) Security software like antivirus that does real-time scanning of files.
I would exclude the program from being able to scan the .PST file type.

2) Does she have any third party applications that integrate with Outlook.
(These would be addins that *might* be listed under Tools Options Other
tab Advanced Options Addin Manager and/or COM Addins.
