Default forwarding custom contact forms

The other person will also need access to the published form definition. Where is the form published? They may need to publish the contact form to their own Personal Forms library using exactly the same name for it as you did.

I created a custom contact form in Outlook to use as a lead tracking system
for my business. When I try to forward (email) a contact which is entered
into the custom form to another person in the office - even if they have the
custom form installed on their computer - the custom form that I emailed
shows up as the standard form with no custom fields. Does anyone have any
advice on how to get the custom form with all its field to another computer
via email?
