In Outlook 2003, I am attempting to export my Contacts to CSV or Excel.

there are NO choices in the white space. The list is blank, as if Outlook is
looking in the wrong place for Mailbox.pab. Any suggestions?

Outlook doesn't use the PAB any more. It uses your Contacts folder. What
type of CSV (DOS or Windows) are you selecting? If you chose a different type
of file, can you see the Contacts then?
