Default Trouble with "File As" on new contacts

Last First order is the problem. Did you restart outlook after making the
change? I've noticed some goofiness with it I you don't restart but its also
buggy and random so it may not help.

The default "Full Name" order should either be "First (Middle) Last" or
"First Last1 Last2".
