Default Outlook shows duplicate names and folders

Without any information on what stopped working the way you think it should
or the steps to reproduce the problem, I can only you suggest that you read
the Help files on how to configure the Outlook Address Book:

Using Outlook 2003. In our contacts we have three folders: 1. “Contact”,
