Had you uninstalled it from the Control panel, Add/Remove programs, it would
have removed the custom forms. Check and see if its still listed in
Add/Remove programs - if so, try to uninstall it.
You may be able to fix it by changing the custom form back to the standard
appointment form - download

Thanks Diane for your fast reponse and help. I used my favorite
uninstaller,
Revo, to thoroughly remove all entries and locations for Windows Live but
your question helped me find that this mapping thing is built into
Outlook.
I'm in over my head since all I wanted was a good and complete calendar
program.
I couldn't find any icon but there are sections on the ribbon for
"location"
and "maps".
