Reply e-mail address?
It sounds like the all 3 accounts (addresses) are aliases for the same
mailbox. Assuming I'm correct, WYSIWYG.
I don't know what's the default Mail Client on your "other systems" but I'd
very much doubt it's OE if she's accessing all three accounts in the client
and is not seeing the same behavior.
In short: OE places the default address (i.e., Pat@xxxx) in the From line
when Replying to or Forwarding a message sent to either of the other
addresses.

When she replies to an e-mail, in the "From:" box it's always mom@xxxx.
I've had her check in property/details that the original e-mail is coming

to Pat@xxx or pat@xxxx.
She is manually changing the From: box to Pat@xxxx but this is a pain to
remember.
On our other systems, the from: is whichever the e-mail is sent to.
