Outlook version on the recipient? Outlook 2003 SP2 doesn't permit that kind of ad hoc one-off form use.

to enter information like customer name & address, there are boxes to check,
and the message portion is to be used to type a message/explaination. I
copied the compose page to the read page and the "send form definition with
item" on the (properties) tab is checked - BUT - when I send the form out the
receiver only sees what I have typed in the message portion - they don't see
any of the fields or other information on the form.
