Default mail message form missing

I ran the Help | Detect & Repair in Outlook. It did not correct the problem.

Under the menu TOOLS -- FORMS -- CHOOSE FORM... then in the STANDARD FORMS
LIBRARY you see a list of the various forms Outlook uses; Appointment,
Contact, Distribution List, etc. One of the forms should be... Message. That
option is not there on my system.

Somehow the IPM.Note disappeared. I cannot open forwarded mail that uses
this form.

Does anyone know how to retrieve the standard Mail Message Outlook form?
