Default Change icon in custom form

Ok, I see. I did it and it says IPM.Contact.PO (I thought I used a note
form, but I apparently used a contact form) But anyway, the icon used to
work.

Let's check to be sure. Add the Message Class field to any table view and
tell us what value you see for that field for items that are using your
custom form. Also tell us what icon they are showing.

I have a custom form in my journal that I created out of a "Note" form.

and all features of my form still work great as before except the icon.

but the default icon for OL 2003 shows. How do I get my icon back? Where
