Default Outlook checklist form

I have sent the form in two ways. 1. Since its a message form you can send it
using the send button 2. Saving it as an oft file and sending them the file
as an attachment to view.

Everyone I have sent them the file have Outlook version 2003. I have SP2
service level. not sure what the others have. Could that be a problem?

I have checked their outlook version and that isn't an issue. Please advice
