Default attachment not being saved in sent messages...

First of all, I'm not saying I know what's causing this. I was just asking a
question. Certainly attachments can be stripped if she's using a server
based email system with any variety of anti-virus implementations.

Secondly, I've never used AVG but in their technical FAQ there's this:

How to setup the auto filtering (based on extensions) feature
The AVG Email scanner provides an option of the automatic attachment
filtering based on extension type (such as dangerous .exe, .scr, .bat, .reg,
..etc). This feature is very important for a general security of the system –
for example when AVG is not up to date.

Instructions :

Please, run the AVG Control Center, choose the Email scanner component and
press the Properties button. Here, please, select the correct plug-in
(mentioned for the email program you use) and press the Configure button.
The above mentioned settings can be specified in the column Remove files
with these extensions (extensions must be divided by a comma so it will look
like: exe,bat,scr)

So, while more of a *feature* than an "adverse effect", it could certainly
be filtering or stripping attachments. I don't know if this works on
outgoing as well as incoming messages but just saying it's a possibility, or
a THOUGHT, if you will.

She may be thinking that, but I've never heard of AVG having adverse
effects in Outlook.
