Default Forwarding Contacts made from custom form internally

You select the Inbox folder from your desktop, right-click it, and choose
properties. When the properties panel comes up, click the Forms tab and then
the Manage button. Set the source selector to the folder where you published
the form, and set the destination selector to the Org Forms Library. Select the
form in the source list and click the arrow that points from the source list to
the destination list. If you can't select the Org Forms Library, then ask the
Exchange Administrator to give you publishing rights to it. But if you saw it
before, then you should be good to go.

Also, how would I go about distributing it to all the users afterwards? One
PC at a time or is there an easier way?

If you publish it to the Org Forms Library, then all the users on your system
have access to it. That is the first place Outlook will look for any form name
it encounters. Of course, if any of your users use caching mode for Outlook,
you probably need to have them clear out their cache files.
