What the FormsAdmin tool does is make a change in the Windows registry, adding new keys to the HKEY_CURRENT_USER\Software\Microsoft\Office\9.0\Ou tlook\Custom Forms key. Microsoft released the tool only for Office 2000, which corresponds to version 9.0 of Office. However, if you have a later version of Outlook, you may have used the FormsAdmin tool to export a .reg file and may have edited the key to replace 9.0 with 10.0 for Outlook 2002 or 11.0 for Outlook 2003, then imported the .reg file to make the change.

To resolve the problem, you can use the registry editor to navigate to the Outlook\Custom Forms key corresponding to your version of Office. There you will see subkeys for Compose and Read. Under each of those keys, delete the value named IPM.Contact. This will remove the reference to the custom form, so that Outlook subsequently will open new contacts in its default contact form.
