Why can some see my calendar in 2003, when no permissions are set
 
There are no permissions setup in Outlook 2003 Calendar, yet people can view
my calendar.

How can you deny permission, when it was never given?

Ask your Exchange administrator to check the permissions at the mailbox level. It sounds like they're too loose.
