Where is my appointment template?
 

Outlook 2003

I note that if I look at tools/forms/choose form/appointment (in the
Standard Forms Libraray): I receive this notice: "Form Load Failed".

Is this why it takes forever for a blank appointment form to be created?

I have deleted the cache, and deleted frmcache.

Whats the fix?

Have you tried clearing the forms cache? Tools | Options | Other | Advanced Option | Custom Forms | Manage Forms | Clear Cache
