I want to send a questionnaire without the disadvantage of a word document
which is that the document is distorted when questions are answered.
Sending a PDF on the other hand is more than most people can handle. I was
told such possibilities (i.e. the form remains intact afetr filling out and
returning) exist within Outlook Express but cannot find them.
Anyone?

You could use a Fill in Form.

The one attached created by Microsoft Word version 2000.

I assume the fill in function will only work if the receiver uses Microsoft
Word

If you know HTML it seems just about anything is possible as far as
creating a web page, e.g. using the Rich Text's Source tab.
This might be the type of thing that Stationery users would be more
familiar with.
