Ask in the body of the message.

See this: http://www.howto-outlook.com/howto/addingsound.htm

That begs the question: why do you want to treat your recipients so shabbily
as to clog their inboxes with something that conveys no information?

For midi-files this is a good question, but other people may be insert
a mp3-file with a voice message to play automatically for blind people.
That could be a good method therefore.

I could go along with that, but most of the blind people I know use
text-to-voice.

Of course, because less people know how to play automatically a mp3
file with their emails ;-)

Well, no, because text-to-voice works for all (or most) applications,
whereas an mp3-based message is useful only in mail.
