Contact form looks quite different after adding a new control?
 
Hi.

I make a new contact using default contact form (Outlook 2003).
Then I choose to edit that default form and add some fields and save that
with different name - contact form looks different - fields become
rearranged.
Is there a way to add fields and keep the initial form's look?

You should make no changes to the General Tab of the Outlook form.
Problems such as you describe are common, if you do. Make all your
customization on a back page, like P.2, and the General Tab will remain
the same.
