Routed Message
 
I downloaded Jay B. Harlow's "Routed Message" form but it appears to
be no differnt than the standard Outlook message form. There is no
code or other custom fields.

I downloaded, extracted the file to a folder then double clicked to
open the oft file.

You probably need to open it with the Tools | Forms | Design a Form command,
