How can I insert a form with drop down menus in outlook?
 
How do I insert a form in an outlook message so that the person receiving the
message can simply click reply and fill in the blanks then send?

In general, that doesn't work, not in the least because you don't know what mail client the recipient has. The exception is when both sender and recipients are working in the same Exchange organization and you can publish to the Organizational Forms library on the Exchange server. In that scenario, you can do data-gathering with a custom Outlook form.
