Outlook Forms
 
I've created a form and published it in my personal forms folder. I also
saved it (oft) on my desktop. But when I retrieve the form and address the
email to my self to test it, it comes across as a blank email. I clicked on
the "send from definition with item.

Outlook Forms
 
Uncheck "send form definition with item," republish, then send yourself a message with the published version.
