custom contact form
 
In creating a custome form, I have created a contact history tab. I include
the date and then a notes section, I pull the field each time from my Freq.
used fields that I created. When I publish and then go to use, when I enter
a date in the first field it replicated in all the date fields, same with the
notes- creates the same note in all of the note field. Why am I getting thie
repetition

It sounds like you have not bound each date/time control to its own
custom field. And, if you are using the Notes control that comes with
the form, you can have only one of those. All other copies will just
show the same contents, and which ever had last had text entered will
rule. You have to create multiple, separate multi-line controls and
multiple, separate custom text fields. Bind each text control to a
separate text field.
