I created a reply form (based on email message) with questions that have
check boxes, text boxes etc. Having created custom fields to do this. Called this form Audit Response I created a form based on the email message called Audit Request; setting the Actions tab so the reply to this Audit Request form calls the Audit Response. The Audit Request is sent; the recipient receives the Request, clicks on reply and gets the Audit Response tab to show up. The recipient fills in the check boxes, etc and sends the form back to the originator. Originator gets a response BUT no information in the response tab is reflected. The form is blank. Any help would be greatly appreciated. jano |

When you created the Audit Response form, did you bind each control to an Outlook property *and* do the same thing for the controls on the read layout?
