When I click on the contact customized form I created, I get this message "A

What exactly are you clicking on? An item you created with the form? Where is the form published? Where is the item stored?

If I look at the contact folder in my list view, I right click on the
contacts form and go to properties then select the form from "when posting to this folder use:". The form is stored under the personal forms library. That still doesn't tell us exactly what you're doing that results in the error message. Also, can you create a new item using the Tools | Forms | Choose Form command and picking the form from your Personal Forms library?

says it can't find the form and contact our Administrator. I don't want to create a new form, it took me hours to create the one I already have. Thank you! I didn't ask you to create a new form. I asked you to create a new item using the form you have already published to the Personal Forms library. Please try this.

You might want to try publishing the form to the contacts folder where you want to use it.
