Can custom form data populate access database
 
I am trying to create an application for users to use to order custom
reports. I have created an outlook form with custom fields. Is it possible
for the data that comes back with the forms populate an access database
automatically?

Yes, but you'll have to write the code to connect to and update the database. The page at http://www.outlookcode.com/d/database.htm has links to several sample forms that do this.
