How do we setup a form for booking PC repairs?
 

We are a small business that sell PC's but are now doing lots of repairs. We
want to know what program to use and how to setup a form that will take
client details and the details of the problems with the PC that need to be
repaired. It must also note the date it was brought in and the date it is
expected to be returned. Ideally it would work with Outlook calander or a
Small Business Server. If it could integrate and send the client an email
this would be great. Please advise.
