Title bar rename
I created a custom form some time ago and published it as "In Car Video
Request". I then created a new, different form using the original form as a template. I used "publish form as" and named it "Video Trouble Report". However, when I select the form, the title bar for the form shows the template name "In Car Video Request" followed by "Video Trouble Report." What gives? How do I get rid of the "In Car Video Request"?
