Default cannot view customized form

I am new to customizing forms and have gone through all of the tutorials and
designed and published a form and changed the default form in the folder.
When I choose new post the form comes up. After filling in the data and
posting it to the folder, it shows a normal post and the data is missing. In
short, I am unable to view the customized form correctly once it has been
posted in the folder.
