I am trying to create a questionnaire form using outlook. I would like to

Who is the audience for this questionnaire? It's quite possible that a custom Outlook form is not the right solution.

The audience is people from my department. I remember being able to link an
HTML form using FrontPage. However, that will require ASP. Is there a way to just get the results of a form and export it out to access automatically? Nope. You'd have to write your own export code or use a third-party tool to get the results into Access.
